As part of our COVID-19 procedures we are requiring all people who attend Advantage Over camps to complete a form. This ensures that you have read the guidelines and understand the relevant procedures and also ensures we have a record of everyone who has attended the sessions, and when, as part of the track and trace guidelines. By completing this form you are agreeing to let us know if you, or any of your household presents with COVID symptoms up to 14 days after attending the tennis sessions.

Has any of your household experienced any COVID-19 symptoms in the past 7 days?
I understand that social distancing measures will be in place at the tennis session
If any of the members of my household presents with COVID-19 symptoms in the next 14 days I will inform Advantage Over
I confirm that: I will not arrive more than 5 minutes before the start of the session, I will leave the site promptly after the session, I will bring a water bottle, snacks, tennis racket and a jacket/jumper (if required) but no other peronal items (unless checked with the coach)
